Hunter S. Thompson Fear & Loathing is an urban ultra event presented by Haight Ashbury Ultra Society, Zombie Runners, and Fat Ass Runners. The event offers two distances and uses a single loop course that begins and ends at Twin Peaks, taking participants on foot past multiple San Francisco tourist locations.

- 50 Miler
- 50K
The event is run in the "fat ass" tradition and is described with the phrase "No Fees, No Awards, No Aid, No Wimps." Organizers provide volunteer-supported aid stations when possible, but aid is not guaranteed. Approximate aid locations by mile are listed as Twin Peaks, Cesar Chavez / 3rd St, Geary & Webster, Grant Ave & Lombard St (Coit Tower), Marina Green, 30th Ave & El Camino del Mar, Lake Merced, Sunset Blvd, Stanyan St, and a return to Twin Peaks.
There is a checkpoint at mile 25; runners must reach it within 4 hours 45 minutes to continue on the 50 mile route, otherwise they are expected to take the 50K route. From 2024, 50 mile finishers are asked to complete an additional 0.67 mile hourglass loop around Twin Peaks after returning to ensure the full 50 mile distance.
