Machete Muertos 200 is part of the Machete Trail Madness Día de los Muertos event at Lake Perris. The event offers multiple race distances and a looped course format that returns runners to a group campground and transition area each lap.

The 200-mile route is described with over 44,000 feet of elevation gain and technical terrain. Course details note a 20-mile loop used by the 20-mile, 100- and 200-mile races with about 4,500 feet of elevation per lap, and a 10-mile loop with about 2,300 feet gain per lap for the 10 and 50 milers. Aid stations are placed on the loops (mile 6 for the 10/50, miles 8 and 13 for the longer loop). The 200-mile cutoff is 96 hours; cutoffs for other distances range from no cutoff for the 10 miler up to 48 hours for the 100 miler.
Pacers are allowed with restrictions by distance and laps completed. The ruck division lists required weights: 20 pounds for females and 30 pounds for men. Group camping is provided on site; family, friends, and pacers may camp without extra charge. Cash podium prizes are listed for the top three overall in the 100 and 200 milers.
