The Cathedral Caverns Trail Run takes place at Cathedral Caverns State Park in Woodville, Alabama. The event features two distinct distances through a landscape of mountains, fields, and a notable cave system.

The course begins in a valley surrounded by mountain views. Both the 5 km and 15 km routes include a final 1.2-mile section that goes through the cave as an out-and-back segment. This path is approximately 0.6 miles deep and wide enough for two-way traffic. Participants receive a glow stick at the turnaround point inside the cave, where decorative lighting remains on. To manage participant density, the 5 km route includes an initial 0.9-mile stretch on gravel and paved roads before transitioning to single-track trails.
The event is designated as a cupless race to reduce waste. Participants are required to carry their own hydration systems, such as handheld bottles, packs, or reusable cups. Aid stations along the course provide water, electrolyte drinks, soda, and snacks like pretzels and chocolate.
The race has specific time limits due to scheduled cave tours. The 5 km has a cutoff of 1 hour and 15 minutes, while the 15 km has a cutoff of 2 hours and 30 minutes. The field is limited to 300 runners for each distance. Participants receive a tri-blend t-shirt and a finisher medal, with awards provided for various age groups ranging from under 19 to over 70.
