The main event features a full 42.195 km course that starts and finishes near the iconic Imperial Forums and the Colosseum. The route is characterized by its historical surroundings, passing landmarks such as Piazza Venezia, the Circus Maximus, the Tiber riverfront, Castel Sant’Angelo, and St. Peter's Basilica.
The event includes several participation options:
- 42.195 km (Full Marathon)
- Run4Rome Relay (Relay for teams of four)
- 5 km (Saturday Fun Run)
The Run4Rome Relay allows teams of four to complete the marathon distance together, with segments of approximately 11 km, 12.5 km, 8 km, and 10.695 km. This format is part of a charity program supporting various non-profit organizations.
On the Saturday preceding the marathon, the Saturday 5k Fun Run takes place. This non-competitive event is open to all ages and does not require a medical certificate. It starts at the Imperial Forums and finishes inside the Circus Maximus.
Participants can access an Expo Village located at the Circus Maximus (Metro B stop) in the days leading up to the race for bib collection and exhibits. The event holds ISO 20121 certification for sustainability. Official training plans and collective training sessions, known as "Get Ready" events, are also provided to help runners prepare for the distance.
